Care Manager

Care ManagerWe are seeking a highly skilled and organized Care Manager with a strong background in social work and/or RN, specializing in post-acute care and senior populations. This role is ideal for a professional who understands the complexities of care transitions, discharge planning, and ongoing support to older adults and their families. The Care Manager is to oversee and coordinate care services for our clients in a home care setting. The Care Manager will serve as the primary point of contact for clients and their families, ensuring personalized, high-quality care that promotes independence and well-being. Additional territories include Sebastian, Port St Lucie and Fort Pierce.Key Responsibilities (but not limited to):Access clients' needs through in-home evaluations and collaborate with families and healthcare providers.Develop and implement individualized care plans tailored to each client's preferences and medical requirementsAssist in training and supervising care staff, including caregivers, home health aides and certified nursing assistantsMonitor service delivery to ensure quality standards are met and adjust care plans as neededAct as a liaison between clients, families, healthcare professionals, third party provider partners and internal teamsMaintain accurate documentation, including care plans, progress notes and compliance recordsEnsure compliance with all relevant regulations, licensing, and safety standardsProvide ongoing support and education to clients and their families regarding care options and resourcesQualifications:Ideal Candidate: Minimum of 5 years of experience in home care, healthcare, or social servicesKnowledge of home care regulations, healthcare practices, and community resourcesPass criminal background (Level 2 AHCA)Excellent interpersonal, communication, and organizational skillsAbility to assess client needs and develop appropriate care plansLeadership skills and experience supervising staffCompassionate, patient, and dedicated to improving clients' quality of lifeCertifications Required: CPREducation: Social Work or related field, Bachelor's degree in Nursing, LPN LicenseCompensation: $20.00 - $28.00 per hourRight at Home's mission is simple...to improve the quality of life for those we serve.
