Physical Therapist

Evaluate patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to their needs. Implement interventions that focus on mobility, strength, balance, and overall functionality. Collaborate seamlessly with nursing staff, physicians, case managers, and fellow rehabilitation professionals to achieve optimal patient outcomes. Guide patients and their families through therapeutic goals, discharge strategies, and home exercise regimens. Maintain accurate documentation of evaluations, treatments, and progress in compliance with facility and regulatory standards. Engage in interdisciplinary team rounds and conferences to ensure holistic and coordinated patient care.

Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution. Valid state licensure as a Physical Therapist. CPR certification desirable; specialized certifications in areas like geriatrics or neurology a plus but not mandatory.

Prior experience in a similar role preferred. Competency in treating patients facing complex medical and mobility challenges. Strong teamwork and communication abilities. Proficiency in digital documentation and clinical charting systems.
