Job Title: Client Care Coordinator

Location: 3800 Forest Drive, Columbia, SC 29204 Job Type: Full-Time Salary: $16.50 - $19.00 per hour (based on experience and performance reviews within the first 90 days)

Job Summary: The Client Care Coordinator will play an essential role in ensuring high-quality service to our clients. This position requires flexibility, a high level of organization, and strong communication skills. The ideal candidate will assist in covering shifts, managing client intakes, assessments, and training new staff. This is a full-time role that includes a combination of office work and field visits.

Key Responsibilities:

• Shift Coverage: Be available to cover shifts due to staffing shortages or emergencies. Must be able to work with all clients.

• Client Intake & Assessments: Conduct new client intakes, initial assessments, and reassessments of existing clients as needed.

• Care Plan Management: Maintain and update care plans to reflect the evolving needs of clients.

• Supervisory Visits: Conduct weekly supervisory visits to clients' homes to ensure care quality.

• Community Engagement: Attend community events and marketing activities with senior care consultants to help raise awareness.

• Training & Support: Provide training to new employees both in the office and on-site. Support and guide new team members.

Required Qualifications:

• At least 2 to 3 years of experience working with seniors in a home setting or facility.

• Strong English proficiency and excellent communication skills.

• Ability to pass a background check and drug screening.

• Reliable transportation and a clean driving record.

• Must be available to work every other weekend.

• Ability to work between 7:00 AM - 9:00 PM, Monday through Friday, and PRN nights as needed.

• Must be comfortable with pets and environments that may have smoke.

• Must work well independently and manage multiple responsibilities effectively.

Preferred Qualifications:

• Prior administrative experience in a home health or home care agency.

• CNA certification is a plus, though not required. Must be able to perform caregiving duties as needed.

• High School Diploma required; College education a plus.
