RN Care Manager - Field-Based, Person-Centered Care

IEHP is seeking a licensed clinician for the Health Access Enhanced Care Management team in Rancho Cucamonga. This position involves providing effective care management to IEHP members, focusing on their medical, behavioral, and social needs.
