Nurse Care Manager

To support a growing healthcare team, the fully remote Nurse Care Manager will manage an assigned patient panel, addressing individual healthcare needs, developing comprehensive care plans, and collaborating with interdisciplinary teams to improve patient outcomes in the Central Time Zone. Key responsibilities Manage overall care for assigned patients using clinical expertise and value-based care principles Build trusting relationships with patients and families, providing clinical and emotional support throughout their care journey Coordinate interdisciplinary care and monitor care plan effectiveness to achieve desired outcomes Required qualifications Associate Degree in Nursing Current RN license required; compact license preferred 3+ years of care management experience Chronic and complex care management experience Intermediate skills with MS Office Suite, including Outlook and Teams
