Requirements:

• Valid and unrestricted Florida Physical Therapist license
• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
• Current BLS certification issued through American Heart Association (AHA)
• Comprehensive background checks and health screenings required
• Immunization records including MMR, Varicella, Hepatitis B, TB testing, and seasonal influenza
• Competency exam and Joint Commission Core I, II, and III certifications
• Florida ACHA fingerprint background check required prior to start date
