(RN/LPN-QBHS) JOB DESCRIPTION

PURPOSE

Under the direction of the Clinical Director or Licensed Therapist, the Registered Nurse/Qualified Behavioral Health Specialist (RN/LPN–QBHS) provides integrated health promotion, prevention, and education services to individuals with mental health or substance use needs. The RN/LPN–QBHS conducts basic health screenings, monitors vitals, and supports clients in understanding and managing their physical and behavioral health. Services are delivered in the office, community, school, or residential settings and are designed to enhance wellness, reduce health risks, promote recovery, and support overall functioning in the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES

​​​​​​​ Health Promotion, Maintenance, Restoration , ​​​​​​​ Prevention & Education

- Conducts routine health screenings and collects basic vitals including blood pressure, pulse, temperature, weight, and other clinically relevant measures.

- Provides client education on wellness topics such as nutrition, chronic disease prevention, medication adherence, stress management, and lifestyle modifications.

- Identifies early warning signs of physical or mental health deterioration and reports findings to supervisory clinical staff.

Client Support & Behavioral Health Interventions

- Provides strength-based, goal-focused interventions consistent with each client’s Individual Treatment Plan (ITP).

- Supports clients in developing coping skills and navigating both physical and behavioral health needs.

- Promotes symptom management, daily living skills, and improved community functioning.

- Assists clients with medical linkage and community resources.

Coordination & Documentation

- Documents all services provided according to agency, OHMAS, and Medicaid requirements.

- Collaborates with the treatment team to review client progress and update plans.

- Reports significant changes in client status immediately to supervisors.

Service Delivery Expectations

- Conducts appointments in the community or office setting.

- Maintains productivity standards and participates in all required meetings and trainings.

- Demonstrates professionalism, teamwork, and reliable attendance.

MINIMUM QUALIFICATIONS

• Current active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Ohio.

• Bachelor’s Degree preferred; Associate degree accepted.

• Eligible to serve as a Qualified Behavioral Health Specialist (QBHS).

• Experience with mental health or substance use populations preferred.

• Proficiency in Google Workspace and electronic records.

• Valid driver’s license, clean driving record, reliable vehicle, and insurance.

• Successful background check.

• Knowledge of nursing assessments and preventive care.

• Understanding of behavioral health symptoms and recovery principles.

• Strong communication and client education skills.

• Accurate documentation abilities.

• Strong judgment, problem-solving, and time management.

• Ability to work effectively in community settings.

• Ability to collaborate within a multidisciplinary team.

CONDITIONS OF EMPLOYMENT

• Satisfactory background investigation.

• Valid driver's license and insurance.

• CPR/First Aid certification (or willingness to obtain).

• Completion of all agency-required training.
