Occupational Therapist
Habitat Health empowers older adults to experience more good days in their homes and communities. Through the Program of All-Inclusive Care for the Elderly (PACE), we provide comprehensive medical care along with support for daily needs such as meals, transportation, and in-home assistance. We deliver coordinated clinical and social care in our centers and directly in participants' homes, creating a fully integrated experience that brings peace of mind and a true sense of belonging. As we expand our scalable, affordable PACE model to meet the growing and complex needs of aging populations, our mission-driven care teams continue to help participants live well on their own terms.
The Occupational Therapist provides clinical occupational therapy evaluation and treatment across acute, restorative, and maintenance levels of care for PACE participants, developing individualized treatment plans, conducting home visits as needed, and instructing participants, caregivers, and staff on maximizing safety and functional independence in Activities of Daily Living (ADLs), exercise programs, and DME use. As a core member of the Interdisciplinary Team (IDT), this role contributes clinical assessments and care coordination in a collaborative spirit that informs whole-person care planning, while maintaining documentation that meets all regulatory requirements. Beyond direct care, the Occupational Therapist serves as a subject matter expert for occupational therapy program standardization, team education, and quality audits, supports DME inventory management, and upholds safety and infection control standards all while contributing to a center culture grounded in Habitat's values and participant belonging.
Core Responsibilities & Expectations for the Role
Participant Care

- Provides occupational therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.
- Participates in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination.
- Assesses condition, creates a treatment plan and discusses implementation of therapy with participants and caregivers.
- Makes home visits as needed and documents activities including participant/caregiver response in participant's chart.
- Educates participants and family members in proper care and use of various supports and exercise programs in addition to the use of DME.
- Documents therapy services provided and performs department record-keeping within documentation requirements
Administration and Program Development
- Continue to raise the bar. Constructively seek and share feedback and help us implement changes to improve clinical outcomes and experience for participants.
- Assists in the ordering, inventory, distribution, and maintenance of durable medical equipment (DME) for participants.
- Maintains the quality assurance, safety, environmental and infection control programs.
- Performs data collection, prepares reports and maintains any necessary participant statistics.
- Attends all staff meetings and patient care conferences as scheduled.
Required Qualifications:
- Masters or Doctorate in Occupational Therapy (MOT or DOT) or equivalent from Accredited Program meeting AOTA & ACOTE standards
- National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association.
- A minimum of one year experience working with the frail or elderly.

Nice to have:
- 2+ years as a practicing OT.
- Bilingual: Spanish/English strongly preferred; additional / other languages a strong plus.
- PACE experience and/ or ADCH/CBAS experience.
- A state issued driver's license, personal transportation, and auto insurance as required by law.

- Remain stationary and/or move about for extended periods, consistent with job duties.
- Operate standard office equipment and/or clinical tools (e.g., computers, phones, medical devices, documentation systems).
- Travel between work sites or community locations as needed.
- Follow safety procedures, infection control protocols, and use personal protective equipment (PPE) when required.
- Perform tasks that may involve bending, reaching, lifting, or assisting participants, depending on role responsibilities.
- Lift, carry, push, or pull supplies, equipment, or materials typically up to 2550 pounds, with assistance or mechanical aids, depending on job duties and role responsibilities.
Compensation: The anticipated base salary range for this position is $110,000 - $120,000. This position is also eligible for a target annual bonus of 5% of your annual salary and may be eligible for additional incentive compensation, as applicable.
The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
In addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance; paid parental leave, short- and long-term disability coverage; life insurance; flexible spending accounts; a 401(k) savings plan that includes a match; paid time off; and company paid holidays.

Employment with Habitat Health is at-will.
