Catholic Charities Brooklyn and Queens seeks a Housing Wellness Coordinator for our Supportive Housing Programs in Brooklyn. This full-time role provides on-site wellness services, counseling, and social supports to residents aging in place, with a mission to help maintain permanent affordable housing. Under supervision of a licensed clinician, you will assess tenants, develop service plans, coordinate care with families and health facilities, and respond to emergencies on site. #J-18808-Ljbffr Catholic-Charities-Brooklyn-and-Queens
