Seven Healthcare are seeking an experienced Physical Therapist for a travel assignment in Bayonne, NJ. This role involves working with patients in a home care setting, applying your expertise to evaluate and treat patients based on physician referrals and orders.

- Establish measurable goals and develop a treatment plan to maximize patient independence/function
- Reassess treatment plan effectiveness and modify as necessary
- Communicate patient needs and progress to treatment team, physician, and family, as applicable
- Consult with medical providers on patient treatment and discharge planning
- Recommend and assist in securing adaptive equipment to enhance patient rehabilitation
- Maintain documentation for evaluations, treatments, and billing as required
- Supervise Physical Therapist Assistants and Aides in patient care activities
- Physical Therapy License in NJ
- Proficient in Home care home base (HCHB), Point care, Point click care
- Experience with Microsoft Teams and strong organizational skills
