United Medical Clinic is seeking dedicated and skilled Nurse Practitioners to join our team at our Rockville Center facility. This full-time role is integral to our mission of providing holistic and value-based primary care to older adults on Medicare. You will work onsite within a school setting, ensuring that our patients lead healthy and fulfilling lives while minimizing hospital visits through proactive and comprehensive healthcare strategies.

Work Environment

This position is based onsite in Rockville Center, New York, within a school setting. You will be part of a collaborative care team composed of a physician, nurse practitioners or physician assistants, medical assistants, and a dedicated medical scribe. Together, you will work cohesively to provide quality healthcare tailored to the school population. Our work schedule consists of weekdays, Monday to Friday, from 9:00 AM to 5:00 PM, allowing for a balanced work-life integration.

Key Responsibilities

As a Nurse Practitioner at United Medical Clinic, your key responsibilities will include:

• Providing exceptional primary care to adults, ensuring their healthcare needs are met efficiently and effectively.
• Conducting routine and acute office visits, employing a patient-centered approach to assess and address health concerns.
• Administering annual wellness visits and comprehensive health risk assessments while maintaining accurate and clear documentation aligned with holistic health practices.
• Coordinating care with other healthcare providers, specialists, testing facilities, and community agencies to ensure a seamless patient experience.
• Leading initiatives in population health by ensuring patients receive necessary evidence-based screenings, such as breast cancer screenings for eligible women.
• Participating in phone triage and outreach initiatives to engage with patients proactively and support their health journey.
• Educating patients and their families about health conditions, care plans, and available treatments to empower informed health decisions.
• Participating in promotional activities that enhance community engagement and awareness of our services.
• Conducting home visits as needed to provide care for patients who may have mobility constraints.
• Assisting the medical team with other tasks as assigned and reporting directly to the Medical Director for operational alignment.

Patient Population / Client Focus

This role is focused on delivering personalized primary care to older adults, primarily those enrolled in Medicare. Your work will ensure that patients feel valued, understood, and engaged in their care. By fostering meaningful relationships, you will play a vital role in addressing their unique health challenges and supporting their overarching well-being.

Qualifications

The ideal candidate will possess the following qualifications:

• A Master's degree in Nursing, a Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner, accredited through either AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license along with an active DEA license.
• Experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs) to properly assess and document patient care.
• A collaborative spirit and a passion for contributing to the advancement of population health initiatives.

Preferred Experience

While not mandatory, candidates possessing bilingual proficiency and a demonstrated ability to work in a cross-functional team environment will be preferred. Experience with innovative healthcare models focusing on quality metrics will also stand out in applications.

Compensation and Benefits

The position offers a competitive salary range of USD 180,000 to 200,000 per year, commensurate with experience and expertise. In addition to the base salary, our comprehensive benefits package includes:
