Requirements

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• CHPN certification - Preferred
• Proof of current CPR certification - Required
• Current driver's license - Required
• Must have reliable transportation and insurance - Required
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required
• 2+ years Home health experience - Required

Responsibilities

• Assess home health patients to identify their physical, psychosocial, and environmental needs.
• Develop, document, and evaluate the Plan of Care to ensure quality of care.
• Educate patients and their families regarding care and promote self-care practices.
• Supervise and provide clinical direction to home health aides to ensure quality of service.
• Perform on-call duties and communicate effectively with patients, families, and healthcare professionals.
• Attend interdisciplinary team meetings and provide appropriate documentation.
