Social Services Consultant - Hybrid, Full Benefits

Join our team as an IRIS Consultant, supporting individuals to live independently and manage their own care through the IRIS (Include, Respect, I Self-Direct) program. In this social services role, you'll build trusting relationships, develop IRIS Support and Services Plans, and assist participants in understanding and maintaining their IRIS eligibility. You will conduct monthly contacts, quarterly visits, and possibly annual Functional Screens, documenting all interactions promptly. The position involves collaborating with local agencies and working independently within a supportive team. Qualifications include a Bachelor's degree in social work or related fields with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience. Bilingual skills are a plus. Candidates should have a strong commitment to participant autonomy, proficiency with smartphones and computer systems, and a respectful, empathetic approach to diverse populations. Requirements: valid driver’s license, reliable transportation, and ability to meet auto insurance standards.
