Home Health Nurse

We are seeking an experienced Registered Nurse (RN) – Home Health to provide high-quality, patient-centered care in the home setting. Experience: Minimum 2 years of RN experience Employment Type: Contract / Travel opportunities available Pay: Upto $51/hr Responsibilities Provide safe, competent, and evidence-based nursing care to patients in their homes Assess patient needs and develop, implement, and evaluate individualized plans of care Collaborate with patients, families, providers, and interdisciplinary care teams Provide nursing interventions including IV therapy, peripheral IV access, lab specimen collection, and wound care Monitor patient progress and ensure care is delivered according to provider orders and the plan of care Educate patients and caregivers on treatment plans, medications, disease prevention, and post-discharge care Manage an assigned caseload and ensure patients progress toward established goals Document patient care accurately in the EMR Identify and elevate safety concerns, barriers to care, and changes in patient condition Participate in quality improvement, evidence-based practice, and patient safety initiatives Requirements Minimum 2 years of Registered Nurse experience BLS certification from AHA or Red Cross Relevant clinical experience and strong nursing assessment skills Excellent communication, interpersonal, critical-thinking, and problem-solving skills Ability to work independently while collaborating effectively with interdisciplinary teams Proficiency with Electronic Medical Records (EMR) and basic data-entry systems Ability to perform clinical procedures such as venipuncture and other nursing interventions Ability to travel to patient homes and meet the physical demands of the role Pride Global offers eligible employee’s comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance, and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance, and employee discounts with preferred vendors.
