Kaiser Permanente is seeking an LMSW Case Manager Social Work professional to join our Healthcare & Medical Services team. In this role, you will provide comprehensive case management for diverse patient populations, coordinating medical, behavioral health, and community resources to support optimal outcomes. Responsibilities include psychosocial assessments, individualized care planning, discharge planning, patient advocacy, crisis intervention, and collaboration with interdisciplinary teams. You will document services in the EMR, ensure regulatory and ethical compliance, and support quality improvement initiatives within a collaborative, patient-centered environment.
