ZURIEL MEDICAL- WHERE EXPERTISE MEETS EMPATHY is seeking a Case Management Nurse for a full-time, remote position. The role coordinates patient care, develops care plans, facilitates safe discharge, and collaborates with physicians and interdisciplinary teams. It requires RN licensure in Illinois or eligibility and CCM certification is a plus.
