Responsibilities
- Conduct comprehensive PT evaluations and develop individualized treatment plans for residents
- Provide rehabilitation interventions to improve mobility, strength, balance, gait, and functional independence
- Administer therapeutic modalities and establish gait training, transfer training, and fall-prevention programs
- Monitor resident progress, re-evaluate as needed, and adjust treatment plans; document outcomes in the chart
- Collaborate with an interdisciplinary team (nurses, aides, OT, SLP, social work) to coordinate care and discharge planning
- Educate residents and families on home exercise programs, safety, and expected goals

Requirements
- Experience in a skilled nursing facility or geriatric rehabilitation setting (preferred)
- Ability to perform accurate evaluations and develop effective treatment plans
- Strong documentation and communication skills; comfortable with electronic health records
- Demonstrated ability to work as part of an interdisciplinary team
- Flexibility to commit to a travel assignment with the listed shift schedule
