Home Health Quality Assurance LVN

Job Summary The Quality Assurance LVN, under the direct supervision of the Quality Assurance Nurse Supervisor, implements the Quality Improvement Program and supportive activities. Review clinical charting for conformance/compliance to Medicare and Agency standards. Participate in quality assurance and improvement activities for the agency. Responsibilities Audits daily notes for proper documentation and ensures chart reviews are timely, accurate, and meet all data requirements under CMS guideline. Monitors compliance with both internal standards and regulatory requirements. Discusses any noted documentation discrepancies with QA RN Supervisor and Clinical Managers. Regularly participates in case conference activities and assists with record reviews that conferences are conducted and documented in accordance with Agency policy. Participates in Quality Improvement (QAPI) Program and assures appropriate corrective measures are identified and implemented. Assists in tracking QAPI items (infections, hospitalizations, incident reports, etc.). Performs other duties as assigned. Required Qualifications Education: Graduate of an accredited Licensed Vocational Nurse program. Experience: 2 years of experience in home health care. Licenses and Certifications: Licensed Vocational Nurse (LVN) current licensure by Board of Nurse Examiners for the State of Texas (or BNE Compact State) upon hire. Preferred Qualifications Experience: 2 years of Home Health Quality Assurance experience.
