Behavioral Health Social Worker: Acute Care Case Manager

Community First Medical Center in Chicago is seeking an experienced Case Manager/Utilization Manager to support the Behavioral Health Unit. The role is full-time and operates 8am–4:30pm, focusing on coordinating care and discharge planning for inpatient populations. Requirements include LSW with LCSW preferred, 2+ years of clinical social work, acute care experience, and a Bachelor's degree. EMR/EPIC experience is preferred.
