Job Summary
At Modani Care, we are seeking an experienced and compassionate Home Health Aid to join our team of dedicated professionals. As a Home Health Aid with Modani Care, you will play a vital role in providing top-notch in-home care services to seniors and adults who require support with daily living activities. Our team helps families navigate the complex process of care, understand their options, and receive personalized support to ensure their loved ones remain safe and comfortable in the comfort of their own homes.
Responsibilities
• Provide high-quality, one-on-one care to clients in their homes, including assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
• Assist with medication reminders, light housekeeping, and meal preparation to support clients' independence and overall well-being
• Develop and maintain positive relationships with clients, families, and healthcare teams to ensure seamless care coordination
• Maintain accurate and detailed records of client care, adhering to all relevant regulations and guidelines
• Collaborate with interdisciplinary teams to develop and implement personalized care plans that meet clients' unique needs and goals
Requirements
• 2+ years of experience as a Home Health Aid or similar healthcare professional
• Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams
• Ability to work independently and as part of a team, with a focus on delivering exceptional client care
• Reliable transportation and a valid driver's license (if applicable)
• Ability to lift, transfer, and reposition clients as needed, with or without assistance
