Registered Nurse (RN)

Job Overview Registered Nurse (RN) or Licensed Practical Nurse (LPN) needed in Portsmouth, OH to administer patient care in accordance with a physician‑established care plan. This is a 1099 independent contractor position. Shifts Multiple shift options—day, evening, night, weekends—and any combination of 12‑hour or shorter visits are available. The role offers flexibility for PRN or full‑time work. Responsibilities Execute physician‑prescribed plans of care. Complete compliant documentation of the care provided in the system of record. Manage and administer prescribed medication, treatment, and therapies. Conduct patient assessments and coordinate care. Perform various activities associated with daily living. Monitor vitals and gastrointestinal intake and output. Assess skin integrity and administer wound care. Monitor for changes in the patient’s condition such as weight loss or gain, self‑care abilities, and indicators of disease progression. Communicate with family members, physicians, case managers, and other designated individuals regarding the patient’s health. Provide education and training to the patient, their family, and the Home Health Aide. Work one‑on‑one in the patient’s home to provide a comfortable environment. Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants. Perform other personal care services as necessary to meet the patient’s needs. Qualifications Graduate of an accredited college or university, associate or baccalaureate School of Nursing. Current State or Compact License as a Registered Nurse or Licensed Practical Nurse. Current CPR certification. TB test and physical examination. Maintain required insurance. Good interpersonal skills. Hand‑eye coordination and manual dexterity. Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients. Ability to operate under stressful conditions and make quick decisions. Dependable transportation and willingness to commute to patients’ homes. Ability to provide basic medical management up to medically fragile end‑of‑life care. Minimum one year experience as an RN/LPN in a clinical or home‑health setting. Ability to assist with two‑person assist. Other Requirements Offers are contingent upon passing a background check.
