Key Responsibilities\n

• Conduct thorough assessments to understand the unique needs of each patient and tailor treatment strategies accordingly.

• Maintain effective communication with supervisors, colleagues, and clients' families to monitor each individual's progress and adapt treatment plans when necessary.

• Develop a tentative schedule of appointments, home visits, and progress monitoring to ensure structured support for each client.
\n
• Coordinate comprehensive home visits, involving chart reviews, staff interviews, and data collection to understand the client’s circumstances in depth.
\n
• Implement and oversee program plans effectively while ensuring measurable outcomes and client satisfaction.
\n
• Participate actively in Interdisciplinary Team Meetings, contributing valuable insights and updates related to patient care.
\n
• Complete detailed monthly and quarterly progress notes that capture the evolving needs and successes of patients.
\n
• Facilitate training sessions for staff and team members on best practices surrounding program implementation and documentation protocols.
\n
• Provide ongoing follow-up through interviews and visits to sustain personalized client support.

Patient Population / Client Focus\n

As a vital member of our team, you will work intimately with a range of clients, including adults and youth, who require varying levels of care pertaining to emotional, psychological, and behavioral health concerns. Our focus includes but is not limited to family dynamics, couples in distress, trauma Survivors, and individuals struggling with substance misuse. Your expertise will greatly influence our clients' journeys toward healing and recovery.

Qualifications\n

• A Master’s or Doctorate degree in a relevant mental health field is mandatory.

• Possession of an active and unrestricted California state license as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW).

• A minimum of two years of counseling experience is required, preferably within a clinical or therapeutic environment.
\n
• Extensive knowledge of legal and ethical standards pertinent to mental health practice is essential.

• Demonstrated ability to collaborate efficiently with various healthcare team members to optimize client outcomes.

Preferred Experience\n

• Specialization in family therapy, couples therapy, trauma-informed care, and/or substance misuse counseling is highly preferred.
