UAS Nurse Assessor (RN)

We are seeking an experienced Registered Nurse (RN) UAS Assessment Nurse to provide vital home health assessments and support community-based care. Responsibilities Include Conducting UAS-NY assessments in patients' homes to evaluate physical, cognitive, and psychosocial needs. Completing accurate assessment documentation and determining eligibility for home care services. Developing care recommendations based on assessment findings. Collaborating with caregivers, providers, and interdisciplinary teams to coordinate care. Educating patients and families on available services and resources. Ensuring compliance with agency and state guidelines. Qualifications Active New York State RN license and current UAS-NY certification. Previous UAS assessment experience required. Preferred background in Home Care, MLTC, Case Management, or Community Health. Strong assessment, communication, and documentation skills. Ability to work independently in the field and remotely. This role offers a hybrid work model with assessments conducted in the Bronx and remote work from home, providing flexibility and work-life balance. #J-18808-Ljbffr Mental Health Association of Rochester
