Access Foster Care and Adoption is hiring a full-time Foster Care Coordinator to provide support for adolescents and children in foster care provider homes. The caseload is in the Lehigh Valley Region with an office base in Bethlehem for weekly team meetings and supervision . This role requires the ability to travel within the Lehigh Valley region. Mileage reimbursement is provided.

Position Summary : Provide support to children in foster care provider homes in the Lehigh Valley. Promote healthy bonding between each child and their foster family through regular visits, training and crisis intervention. Work with the child’s treatment team to develop and implement individualized service plans that address each child’s social, emotional, educational, and physical needs and promote community belonging. Coordinate and oversee the implementation of all services outlined in the service plan. Help ensure each child’s safety and well being through regular monitoring and through implementation of all agency, state and county regulations.

Foster care coordinators will also receive training to complete SWAN adoption cases.

Essential Functions

• Develop, implement, update and review individual service plans that are strengths based and person centered in collaboration with the children being served and the team.
\n
• Coordinate the implementation of all supports outlined in each child’s plan.
\n
• Collect and prepare program and/or individual related data as requested.
\n
• Connect individuals to resources and supports to the community.
\n
• Promote, support and encourage current trend/philosophies within the human services field in accordance with Access Foster Care and Adoption’ philosophies.
\n
• Participate in departmental meetings, trainings and activities.
\n
• Support and monitor provider homes to help ensure that they comply with county and state regulations.
\n
• Conduct foster home safety inspection as assigned.
\n
• Assist with the orientation of new providers and assure that all providers receive appropriate training.
\n
• Participate in the overall referral process including the assessment of referrals, interviewing prospective individuals and overseeing the matching process.
\n
• Maintain a thorough knowledge of program licensing regulations as well as CAASP principles.
\n
• Coordinate and assure that adequate respite care services are provided.
\n
• Supervise visits with child and family as required by court/county.
\n
• Develop and implement goals toward reunification of the child and his/her natural family according to the procedures outlined in the Adoptions and Safe Families Act.

Requirements and Qualifications

Selected candidates must meet all qualifying and ongoing job criteria including background checks, physical examination results, and driving records.

Driving Requirements: This position requires regular driving during the workday throughout the Lehigh Valley region.

To be eligible, candidates must:

• Possess a valid U.S. driver’s license

• Have reliable access to a personal vehicle
\n
• Maintain an acceptable driving record in accordance with the Motor Vehicle Policy
\n
• Be willing and able to drive as part of regular job duties
\n
• Successfully pass all motor vehicle record checks
\n

Education: Bachelor’s degree required, preferably in human services, social work, psychology, education or criminal justice. Master’s degree in a related field preferred.

Experience: Two (2) years of direct experience with children/adolescents and/or case management experience preferred.

• Proficiency in, or ability to learn and use, workplace applications, platforms and technologies including the electronic healthcare record system, and time and attendance system.
\n

Essential Working Conditions/Physical Demands: Work in program sites and the community routinely providing direct care and/or support with individual(s). Stand, sit or run by self, or to provide proper care and supervision with an individual. Able to move and stay with an individual to keep them safe, both in home environment and community, both at near and far distances, even when an individual may struggle to meet identified expectations. Able to assist an individual who has fallen, regardless of weight or size. Walk, climb stairs by self, or to provide direct care and/or support. Work in a standard office environment. Utilize desktop computer equipment on a daily basis. Able to lift or move items associated with sedentary work and light work (exert up to 20 lbs.) Travel to offsite locations and regional office locations if necessary.

Compensation, Schedule, and Benefits:

Schedule: General working hours are Monday-Friday: 8:30am-4:30pm with the ability to be flexible and work weekends and evenings as necessary to meet with children and provider families. Foster Care Coordinators participate in an on-call rotation.

Compensation: The hourly rate for the Foster Care Coordinator is $20 per hour.
