Job Summary:
Physical Therapist Home Health (PT HH) position located in the relevant healthcare setting. This role involves providing patient-centered therapy services in a home health environment, focusing on rehabilitative care for adult and geriatric patients. The position requires delivering high-quality patient assessments and therapy interventions in accordance with established clinical guidelines and standards.

Job Details:

• Schedule: 8-hour shifts, minimum 40 hours per week, beginning on 09/14/2026 and ending on 12/14/2026.
• Required Qualifications: Valid California Physical Therapist license, at least 1 year of experience as a Home Health Physical Therapist, current BLS certification. Valid drivers license and auto insurance are also required.
• Preferred Qualifications: BLS certification (American Heart Association) is preferred.
• Additional Information: The role entails a patient ratio of 5 visits per day, primarily working with adult and geriatric patient populations. Experience with charting systems such as Community Hospital EMR is advantageous. Candidates are expected to adhere to all clinical and operational policies, including patient assessment, treatment planning, and documentation. Flexibility in clinical procedures and proficiency in a variety of therapeutic skills are highly valued.
