Occupational Therapy - OT SNF
Travel Occupational Therapist opportunity in a Skilled Nursing Facility setting for a 13-week contract with 35 guaranteed hours per week. This role focuses on helping patients improve functional independence, mobility, and daily living skills within a rehab-focused SNF environment.
Responsibilities include patient assessments, individualized treatment planning, therapeutic exercises, ADL retraining, balance and coordination training, adaptive equipment recommendations, transfer training, patient and caregiver education, discharge planning, and interdisciplinary collaboration. The OT will document patient progress, modify treatment plans as needed, and support safe rehabilitation outcomes and improved patient independence within the SNF setting.
