Job Summary and Qualifications The Case Manager Assistant will support the Case Managers & Social Workers by carrying out utilization and discharge planning activities as assigned. This will include, but not be limited to communicating clinical information to payers for hospital certification; tracking educational requirements of patient’s/families & assuring they are scheduled and completed; communicating referral information to post-hospital service providers & running worksheets/rounds reports.
