Care Manager

Job Overview: The role of the Care Manager is to work collaboratively with individuals with intellectual/developmental disabilities, their families, OPWDD, and relevant providers to ensure that all individuals supported are able to live safe, happy, and fulfilling lives. This includes working to identify goals, building plans to reach those goals, and assistance with applying to a variety of supports and services to put the individual on the right path to meet their person-centered outcomes. Tri-County Care supports members of all ages – this may include advocating for individuals with their schools, helping them to find medical providers, assisting them with applications to Social Security, SNAP benefits, Waiver programs, etc. Care Management Overview Video (Please Watch Before Applying) To ensure applicants have a clear understanding of the Care Management role and expectations, please watch the brief overview video linked below before completing your application. Care Management Overview Video: As part of the hiring process, applicants may receive a short questionnaire connected to the overview video to help us assess understanding of the role. Location: This position is remote but does require periodic in-person visits . Candidates will need to be able to travel to do these visits via car or public transportation. Travel reimbursement is provided. Conflict-management and problem solving skills.
