Admission Coordinator Registered Nurse (RN)

The Admission Coordinator Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The CLC Admission Coordinator is an active member of an interdisciplinary team whose primary focus is on the CLC screening and placement processes. They have a keen understanding of the admission criteria of each CLC program area. Collaborates with the members of the healthcare system and stakeholders to provide a seamless, timely referral system to Veterans seeking admission to the CLC. Each referral will be screened and reviewed for disposition and planning. Collaborates with both referring and accepting staff ensuring that Veterans are placed in the program area that can best meet their needs. Collects and maintain data that will enhance program planning and placement. Initiates consultation for complex administrative and clinical issues. The CLC Admission Coordinator assists with patient planning assessments, identification of barriers, and activities required to successfully transition the patient into the CLC setting. Applies the nursing process which includes systematic data gathering, assessment, and appropriate nursing judgment to ensure positive patient outcomes. Monitors bed availability and serves as the point of contact for the outcome of screening decisions. Coordinates and collaborates with other VA and non-VA departments, community agencies, and healthcare providers to ensure patient needs are addressed. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective in discussions with the interdisciplinary team. Partners with others to effect change and produce optimal outcomes. Supports colleagues through knowledge sharing to provide safe, quality nursing care. Shares educational findings, experiences, and ideas with peers. Questions clinical practices for the purpose of providing evidence-based care. Participates in activities and strategies to sustain evidence-based-practice.

Preferred Experience: BSN 3 years clinically complex skilled nursing with hospice, dementia, and rehabilitation. Admissions coordination and discharge planning.

Work Schedule: Monday thru Friday, 7:00am -5:30pm.(4 shifts).
