Responsibilities As a Care Coordinator, you will collaborate closely with the Area Director of Operations, providing essential administrative support within communities. Your primary tasks will include optimizing therapists’ schedules, assisting in building caseloads, performing intakes, conducting care coordination touchpoints, and hosting wellness trainings to ensure consistent interactions with residents, families, and staff. The ideal candidate is highly organized, detail-oriented, and capable of multitasking to support operational efficiency and quality of care within the community. Your essential duties include: Identifying Seniors' Health Needs Proactively engage with senior living community residents to assess therapy needs and advocate for care coordination services. Relay resident needs to Clinic Director and/or ADO to begin screenings. Maintain an on-site presence to cultivate relationships with residents, team members, and community partners. Intake Process Support Clinic Directors in completing the intake process for new residents by collecting necessary documents and obtaining consent from residents or their Power of Attorney (POA). Help in scheduling evaluations and assessments conducted by clinical staff. Assist in acquiring orders and required documents from physicians. Client-Facing Services Deliver care coordination services through regular touchpoint visits. Conduct health screenings under licensed therapists’ supervision, in compliance with relevant regulations. Organize wellness classes, educational events, and personal fitness training. Scheduling and Team Member Support Assist Clinic Directors in organizing therapists’ weekly schedules for optimal efficiency. Support the onboarding process for new team members. Serve as an interim Clinic Director to cover for PTO or staffing vacancies. Reporting Review operational reports and address any gaps in care coverage. Distribute weekly census spreadsheets to relevant team members. Assist with weekly billing closeouts. General Administrative Support Handle various administrative tasks as assigned, including supply coordination and task completion. Ensure compliance with HIPAA and PHI guidelines, maintaining confidentiality at all times. Communicate professionally with physicians, care staff, and interdisciplinary teams. Exhibit flexibility to meet the needs of patients and the community. Other Duties Perform other duties as assigned, consistent with qualifications and industry standards. Qualifications Certification in healthcare, business administration, or a related field; two years of relevant experience in a senior living setting is preferred. Experience in administrative and operational roles, including managing schedules and records. Background in a customer‑facing position, with a focus on providing exceptional service to residents and their families. Familiarity with wellness offerings, including physical, occupational, and speech therapy for seniors, as well as knowledge of Assisted Living, Independent Living, and Memory Care regulations. Exceptional communication and interpersonal skills, along with a genuine passion for working with older adults. Ability to collaborate effectively and take direction from management. Self‑motivated with a proactive approach and a desire to learn and recommend solutions to challenges. Capacity to remain calm in stressful situations, handle multiple interruptions, and excel in multitasking. Respect for patient and resident rights and confidentiality. Computer Skills Proficiency in internet browsers (e.g., Chrome, Edge) and advanced knowledge of Microsoft Office programs, including Outlook, Excel, and Word. Work Environment & Physical Demands This role requires physical capabilities to lift/move 20 lbs, maintain a stationary position, maneuver freely, operate equipment, and communicate effectively throughout the organization. Regular tasks include standing, sitting, walking, using hands and fingers, and reaching. Vision requirements include close vision, distance vision, and depth perception.
