Hiring Range: $25 to $27.50 per hour, paid bi-weekly

The Service Coordinator works under the direction of the Associate Director of Resident Services and in partnership with members of the Property Management team to reduce chronic homelessness and incidences of eviction among CHT’s guests, residents, and tenants by coordinating access to critical health and wellness services and housing resources for individuals and families who are currently experiencing homelessness, or were formerly homeless and currently housed at Champlain Housing Trust. The Service Coordinator will take a proactive approach to engaging with assigned individuals, using empathy, tact, and professionalism to build trusting relationships that promote participation and result in participant access to long-term successful housing placement.

Note: Non-standard, alternative work hours may be required.

Essential Responsibilities

• Work as a site-based Service Coordinator for households living in CHT rental properties, with a focus on households with complex needs, such as those with histories of trauma and/or homelessness.

• Support community building by utilizing various methods. Minimum of one monthly event required to foster resident connections, social space, and reduce social isolation.

• Proactively contact assigned individual guests, residents and/or tenants, using personal introductions, door knocking (or other techniques) to promote successful long-term housing placement.

• Build trusting relationships with guests, residents, and/or tenants to develop a comprehensive knowledge of each individual.

• Work with guests, residents, and/or tenants to ascertain needs and assist in identifying and accessing applicable services offered by CHT or partner agencies as appropriate to prevent eviction and homelessness.

• Check in with assigned residents and resident communities, and be available for support, conflict resolution, and crisis intervention as directed by the Associate Director of Resident Services.

• Provide resources to help guests, residents, and/or tenants resolve social interaction challenges in consultation with the Associate Director of Resident Services.

• Provide education to participants to help individuals/families become self-sufficient.

• Work in collaboration with Property Managers to resolve tenant conflict/lease violations.

• Take part in Resident Services/Service Coordinator duties at other assigned sites to cover the gradual migration of households to other locations in the portfolio.

• Contribute as needed to the Resident Services team to collaborate at other CHT properties.

• Act as an advocate for guests, residents, and/or tenants as appropriate.

• Attend meetings with partner agencies and collaboration groups as required.

• Perform timely and accurate data entry to track statuses of each assigned individual or household progress.

• Work on special projects as needed.

• Other duties as assigned.

Work Atmosphere

Champlain Housing Trust is a non-profit, member-based organization that serves a diverse population in northwest Vermont. The work entails significant self-motivation. Regular travel between sites will be required. Occasional travel to meetings and seminars may be required.

Qualifications

• BA in Social Work, Psychology, or another Human Services field or equivalent combination of education and experience.
• Commitment to social justice, equity, and CHT's model of permanently affordable housing is required.
• Previous experience working with high-risk populations, and/or coordinating services specifically for the homeless population is preferred.
• Knowledge of area resources and programs available to low-income individuals, including transportation, health services, and housing are strongly preferred.
• Ability to perform work with empathy, consideration, respect and professionalism under stressful circumstances is essential.
• High level of self-motivation.
• Familiarity with HIPAA regulations and ability to handle sensitive information in a confidential, professional manner.
• Valid driver’s license and reliable transportation are required.

Physical and Mental abilities: This individual must have the physical and mental abilities to perform the essential job duties listed above.
