We are seeking a dedicated and compassionate Occupational Therapist to join our interdisciplinary team at Hope Creek Nursing and Rehabilitation Center in East Moline, Illinois. As a valued member of our team, you will play a crucial role in helping our patients achieve their maximum potential and maintain their independence.
About the Role:
As an Occupational Therapist , you will be responsible for developing and implementing individualized therapy plans to help patients recover from injuries, illnesses, or surgeries. You will work closely with our interdisciplinary team to identify and address patients' physical, cognitive, and emotional needs, and provide education and training to patients and their families to promote independence and self-care.
Responsibilities:
• Conduct initial assessments and evaluations to identify patients' physical, cognitive, and emotional needs
• Develop and implement individualized therapy plans to address patients' needs and goals
• Provide occupational therapy interventions, including exercises, training, and education
• Collaborate with our interdisciplinary team to identify and address patients' needs and promote continuity of care
• Document patient progress and outcomes in accordance with regulatory and clinical standards
• Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:
A master's degree in Occupational Therapy from an accredited program
Current licensure in Occupational Therapy in the state of Illinois
