Job Description
Title: Registered Nurse - Home Health

Schedule: 8-hour shifts

Assignment Details:
- Contract Length: 14 weeks
- Guaranteed Hours: 40 hours per week

Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software experience
- OASIS experience
- CHPN certification (preferred)
- Proof of current CPR certification
- Current driver's license
- Must have reliable transportation and insurance
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience

Roles & Responsibilities:
- Assess home health and/or hospice patients to identify their physical and psychosocial needs.
- Implement and evaluate the Plan of Care (POC) to ensure quality care.
- Educate patients and families on care, medication, and safety practices.
- Supervise and provide guidance to nursing assistants and other staff.
- Monitor cases for compliance with third-party payer requirements.
- Provide effective communication among patients, families, and interdisciplinary team members.
