Registered Nurse – Home Health

Schedule: 8am to 4pm Tuesday - Saturday or Sunday - Thursday with some on-call and holidays required

Assignment Details: Guaranteed Hours: 37 Contract Length: 13 weeks

Requirements: OASIS experience required Active nursing license in the state Knowledge of State and Federal regulations and VNAHSR policies

Roles & Responsibilities: Utilize the nursing process to assess patient and caregiver needs to provide quality care in accordance with evidence-based practice and regulations. Develop and communicate a realistic plan of care, ensuring patient/caregiver involvement. Serve as Case Manager, actively managing case loads and visit times according to agency standards. Complete OASIS documentation and ensure adherence to documentation standards and timeliness. Implement medical plans of care and nursing interventions while maintaining infection control standards. Collaborate with LPNs to follow the care plan and ensure patient needs are met. Evaluate the effectiveness of the implemented plan of care and document findings accordingly. Educate patients and caregivers on treatment options and care procedures, ensuring informed decision-making. Coordinate with other healthcare disciplines to address interdisciplinary needs of patients. Ensure timely completion of necessary forms related to patient care. Advocate for patient care issues to ensure sensitive and responsive care.

Compliance / Notes: Participates in monthly peer review audits and accepts feedback professionally. Coordinates safe patient discharges and transfers, ensuring appropriate documentation.
