Advanced Practice Provider — Primary Care

Are you a Nurse Practitioner or Physician Assistant looking for a primary care role where you'll build real relationships with your patients — not just see them once in a walk-in? Thundermist Health Center is looking for Advanced Practice Providers to join our primary care teams across Rhode Island. You'll manage your own patient panel, deliver continuity-focused care including preventive, acute, and chronic disease management, and work within an integrated care model alongside physicians, behavioral health clinicians, care managers, and community health workers. This is not a Convenient Care or walk-in role — it's primary care with a panel, a team, and a community that depends on you. Thundermist is a multi-site FQHC serving over 65,000 patients, and we're committed to delivering care regardless of insurance status, immigration status, or ability to pay.

What You'll Do

Clinical Care & Panel Management

• Provide primary medical care to an assigned patient panel including scheduled visits, acute care, chronic disease management, and preventive care
• Perform comprehensive health histories, physical examinations, and diagnostic assessments; order, interpret, and follow up on labs, imaging, and testing
• Prescribe medications, including controlled substances, within scope of practice
• Provide anticipatory guidance and patient education, including pediatric well-child guidance where applicable to specialty
• Coordinate referred services including specialty care, pharmacy, and ancillary services
• Triage patient concerns and determine appropriate level of care, escalation, or transfer

Team-Based Care & Collaboration

• Collaborate with physicians, other APPs, nurses, MAs, behavioral health clinicians, care managers, community health workers, and social work staff
• Support warm hand-offs and integrated behavioral health workflows within primary care
• Partner with care management for high-risk and complex patients
• Coordinate with community agencies and social service partners to address social determinants of health
• Supervise and precept NP, PA, and medical students as assigned

Quality, Documentation & Compliance

• Complete clinical documentation in the EHR (eClinicalWorks) accurately and within same-day charting requirements
• Support HRSA UDS reporting and value-based care performance through consistent care gap closure, screening completion, and preventive care delivery
• Participate in peer chart review, quality improvement initiatives, and clinical care improvement efforts
• Maintain practice consistent with HRSA, OSHA, HIPAA, CLIA, and applicable state and federal regulations
• Maintain credentialing, licensure, national certification, DEA registration, and BLS/ACLS/PALS/NRP as applicable

What We're Looking For

You're a provider who chose primary care because you want to know your patients by name, manage their care over time, and be part of a team that wraps around the whole person. You're clinically sharp, compassionate, and motivated by Thundermist's mission.

• Master's degree from an accredited NP program with Rhode Island APRN licensure, OR Master's from an ARC-PA accredited PA program with Rhode Island PA licensure
• National certification in Family Medicine, Internal Medicine, Women's Health, or Pediatrics (ANCC, AANP, or NCCPA)
• Current DEA registration and Rhode Island Controlled Substance Registration
• Current BLS certification; ACLS, PALS, NRP as applicable to specialty
• Eligibility to enroll with Medicare, Medicaid, and commercial payers (current CAQH profile required)
• Working knowledge of electronic health records (eClinicalWorks preferred)

Preferred:

• Prior experience in an FQHC, community health center, or safety-net primary care setting
• Bilingual (Spanish/English or Portuguese/English) strongly preferred
• Experience with team-based care models, integrated behavioral health, and value-based care
• Experience with panel management, quality metric performance, and preventive care outreach
• Eligibility for NHSC or Rhode Island state loan repayment programs

Schedule & Locations

Full-time (40 hours/week), benefit-eligible. Typical schedule is Monday through Friday during clinic hours, with some evening clinic sessions to support patient access. Clinic hours across all locations are Monday through Friday, 8:00 a.m. to 8:00 p.m. Occasional float coverage across other Thundermist sites may be required.
