What You'll Do:

• Deliver skilled, compassionate in-home therapy as part of a collaborative care team

• Evaluate and develop individualized treatment plans for adult and geriatric patients

• Serve as a key liaison between patients, families, and interdisciplinary team members

• Document care clearly and thoroughly in our EMR system

• Help shape the future of in-home rehabilitation

What You Bring:

• Doctor of Physical Therapy (DPT) or equivalent (MPT, MScPT)

• Current and unrestricted Virginia Physical Therapy license

• Two or more years of experience in home health, acute care, or rehab preferred

• Excellent communication and critical thinking skills

• Valid driver's license, active auto insurance, and CPR/BLS certification
