MDS Nurse (RN)

The MDS Nurse (RN) is responsible for coordinating the Resident Assessment Instrument (RAI) process, completing accurate and timely Minimum Data Set (MDS) assessments, and collaborating with the interdisciplinary team to develop comprehensive, individualized care plans. This position plays a critical role in supporting regulatory compliance, quality outcomes, reimbursement accuracy, and person-centered care while ensuring residents receive services that reflect their clinical needs.

Key responsibilities include coordinating and completing accurate, timely MDS assessments in accordance with CMS, Medicare, Medicaid, and state regulations; collaborating with the interdisciplinary team to develop, implement, and update individualized resident care plans; reviewing clinical documentation to ensure assessment accuracy, regulatory compliance, and appropriate reimbursement; monitoring MDS schedules and ensuring all assessments are completed within required timeframes; analyzing resident conditions and documentation to accurately capture clinical complexity and functional status; participating in Medicare meetings, care conferences, quality assurance activities, and interdisciplinary team meetings; educating nursing staff on documentation requirements and best practices that support accurate resident assessments; assisting with survey preparation and maintaining compliance with CMS, state, and facility standards; identifying opportunities to improve documentation, quality measures, and reimbursement outcomes; and fostering a collaborative, resident-centered approach to assessment, care planning, and clinical excellence.

Required qualifications include a current registered nurse (RN) license in the State of Kentucky, current CPR certification (or ability to obtain within required timeframe), minimum of two (2) years of nursing experience in skilled nursing, long-term care, rehabilitation, or a related healthcare setting, working knowledge of the MDS/RAI process, care planning, and Medicare/Medicaid regulations, strong understanding of clinical documentation and reimbursement principles, excellent communication, collaboration, and organizational skills, ability to manage multiple priorities while meeting regulatory deadlines, strong attention to detail and commitment to documentation accuracy, ability to work independently while collaborating effectively with an interdisciplinary healthcare team, and ability to meet the physical demands of the position.

Preferred experience includes previous experience as an MDS Coordinator or MDS Nurse in a skilled nursing or post-acute care setting, RAC-CT certification or willingness to obtain, experience with PDPM, Medicare reimbursement, Quality Measures, Five-Star Quality Rating System, and regulatory compliance, experience with electronic medical records (EMR) and MDS software, knowledge of Quality Assurance and Performance Improvement (QAPI) processes, strong analytical, problem-solving, and critical thinking skills.
