Outpatient Physical Therapist

The Outpatient Physical Therapist is responsible for evaluating assigned patients, developing, implementing, and modifying treatment plans according to patient needs, goals, and physician directions. The role includes overseeing discipline-specific therapy services and ensuring the design of functional treatment programs focused on effective patient return to community activities. The physical therapist will also serve as an educator for patients, families, students, and coworkers, while adhering to the organization's mission and core values.

Responsibilities:

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Maintain accurate and timely medical record documentation.
• Participate in student program assignments, providing supervision and evaluations.
• Create and pursue an individualized professional development plan.
• Adhere to State Practice Act and complete all state-mandated continuing education requirements.
• Engage in organizational performance improvement initiatives.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for physical therapy.
