Licensed Vocational Nurse (LVN)

The Licensed Vocational Nurse (LVN) assesses, plans, and implements nursing care for residents in a skilled nursing setting under the supervision of a Registered Nurse and Director of Nursing. This role involves administering medications while adhering to the eight medication rights to minimize the chances of errors. The LVN ensures continuity of care by providing direct care and supervising CNAs and support staff, participates in teaching residents and families, and advocates for residents in alignment with the facility's mission.

Roles & Responsibilities

• Assess resident conditions and nursing needs, prescribing appropriate nursing actions.
• Effectively assess and reassess pain while utilizing pain management techniques and educating residents and families about pain management.
• Document changes in residents' conditions and nursing care accurately in progress notes and care plans.
• Perform waived testing (point-of-care testing) per established policies and procedures.
• Interpret waived test results and take appropriate action based on outcomes.
• Administer medications while adhering to the eight medication rights to decrease the potential for medication errors.
• Comply with current documentation standards and facility policies.
• Notify physicians about changes in residents' conditions and ensure appropriate follow-up actions are taken.
• Execute physician's orders promptly and accurately.
• Provide thorough reports to the oncoming shift through walking rounds and proper documentation for upcoming care needs.
• Address any resident concerns based on information received during shift reports or from residents and families.
• Implement resident care plans based on assessments and interdisciplinary team goals.
• Respect and maintain the dignity of residents and their families, addressing their psychosocial, cultural, ethnic, and religious/spiritual needs.
• Supervise healthcare team members as necessary to fulfill care plans.
• Demonstrate technical and manual skills in accordance with LVN scope of practice.
• Act as a resident advocate to protect residents' rights.
• Exhibit creativity in problem-solving and viewing situations from innovative perspectives.
• Engage with residents, families, and colleagues in a respectful and knowledgeable manner.
• Involve residents and families consistently in care planning, treatment pathways, education, and discharge plans.
• Provide leadership to CNAs, delegating tasks that support the nursing unit's success.
• Participate in the orientation of new personnel and assist in performance evaluations as necessary.
• Adhere to proper infection control protocols and supervise staff in maintaining these procedures.
• Demonstrate composure during emergencies and stressful situations, ensuring a sense of stability within the team.
• Consistently follow facility policies and procedures.
• Show genuine interest and appreciation for adult residents in their later years of life.
• Exhibit motivation and independence in managing workload effectively.

Qualifications & Skills

• Graduate of an accredited LVN program.
• Current California Licensed Vocational Nurse.
• Current BLS certification.
• Ability to read and communicate effectively in English; additional language skills preferred.
• Basic computer proficiency.
• Experience with Electronic Health Records is preferred.

Shift Information

Part-time night shift on Wednesdays and Saturdays.

Minimum Years of Experience

2 years of skilled nursing experience required.
