We have an exciting opportunity for a full‑time, Outpatient Mental Health Clinician to join our Gracepoint partial hospitalization program. The Partial Hospitalization Program (PHP) Outpatient Clinician provides individual, group and family psychotherapeutic, supportive and educational counseling, comprehensive care planning and concrete support services in the setting of a partial hospitalization program and intensive outpatient program focusing on mental health and substance use recovery. Demonstrates an understanding of, and embraces, the mission statement of Christian Health. Competencies: Conducts psychotherapeutic counseling and psychoeducation to consumers with a primary mental health diagnosis including those in recovery from a substance use disorder. Provides group, individual and family counseling. Assists in maintaining accurate, legible timely documentation, as per PHP policy, regulation, licensing and accreditation requirements. Conducts comprehensive initial evaluations and assessments in collaboration with psychiatric provider in a timely and accurate manner. Provides, with patient consent, support, education and assistance to families regarding patient's treatment, and discharge, including information and referrals required for appropriate discharge. Seeks and utilizes resources of community agencies, referring patients to available, appropriate community resources and services. Ensures continuity of care with collateral providers, community organizations and referral sources. Assesses discharge planning needs to facilitate implementation of effective transition plan in a timely & accurate manner. Assist in screening referrals together with PHP Interdisciplinary team. Serves as a clinical resource to PHP interdisciplinary team. Attends daily interdisciplinary treatment team meetings. Contributes to the PHP Utilization Review process. Utilizes behavior measurement scales. Participates in the program's Quality Improvement Activities. Provides referral and linkage to community resources and services. Crisis intervention, risk assessments and safety planning. Develops and implements individualized treatment plans in collaboration with the psychiatric provider. Performs case management duties as indicated and clinically determined. Contributes to maintaining the curriculum by supporting review and updating of program content ensuring all material is current, relevant and evidence based. Clinical supervision provided on site to those seeking hours toward LCSW licensure. Experience: Working in a mental health setting or related work experience. Schedule: 8am-4pm, Monday - Friday. Education: MSW from an accredited school of social work required and LSW or LCSW preferred OR Master's degree in counseling or related field from an accredited institution and LAC or LPC preferred. QUALIFICATIONS/REQUIREMENTS

Demonstrates compliance with the CH ASPIRE Standards of Performance. Qualifications: Working in a mental health setting or related work experience. LSW, LCSW, LAC or LPC is preferred.
