Pharmacist Inpatient

Pharmacist Position

The pharmacist ensures the safe and effective use of medications by verifying medication orders and assessing optimal drug product selection, dosage regimen design, effective administration, and drug therapy monitoring. The pharmacist monitors inpatient and outpatient drug therapies and collaborates with medical staff to identify alternatives when needed to ensure the best possible care.

Responsibilities

Receive and interpret medication orders; transcribe to computerized patient medication profiles

Compound and dispense pharmaceuticals, including sterile chemotherapy and parenteral nutrition products

Issue controlled substances to patient care areas and maintain records

Complete pharmacokinetics consults during shift, determining appropriate medication dosing

Provide medication information and education to patients and families

Offer clinical consultation and clarification to practitioners

Suggest appropriate, cost-effective therapeutic alternatives to medical staff

Qualifications

Required:

Graduate of a School of Pharmacy (PharmD or BS Pharm) accredited by the Accreditation Council for Pharmacy Education (ACPE)

Licensed with the State of Kansas Board of Pharmacy

Knowledge of principles, methods, and techniques involved in performing general pharmacy services and adapting/modifying standards of pharmacy practices for specific medical cases

Infection Control

Initial and ongoing training in infection control. Trainings may include, but are not limited to, bloodborne pathogens, bodily fluids, and biohazardous materials as applicable to the daily work environment.

Coding Information: ICD-9 Codes, Rev Codes, CPT Codes

Clinical Information: Diagnosis, Reports/Medical Notes, Test Results, Problem List, Procedures, History and Physical
