Social Services Practitioner (MSW) (Home Health)

Job Description Our home health and hospice agencies are located throughout California, Hawaii, Oregon and Washington. They serve the entire community and offer personalized in‑home care, and many have been recipients of Home Care Elite awards. Job Summary Provides social work support to patients in their place of residence. Responsibilities include providing community resources, placement options and counseling to patients and families as needed. Acts as a competent clinician who provides clinical care in a skilled and effective manner, utilizing appropriate technical and decision‑making skills. Job Requirements Education: Bachelor’s Degree or equivalent (some certifications will require Bachelor’s) – Required. Master’s Degree in social work or equivalent combination of education/experience – Required. Three years’ experience in a social agency, hospital, or other organization dealing with health and/or welfare – Preferred. Licenses/Certifications Licensed Clinical Social Worker (LCSW) in state of practice – Preferred. Valid Driver’s License (DL) and must be at least 21 years of age or older – Required. Essential Functions Makes the initial patient assessment as ordered. Documents assessments, visits, and contacts on patient’s chart. Places phone calls and makes visits as needed, serving as a resource liaison when appropriate. Provides patients/families with community resources. Attends weekly team conferences.
