Contract | Days | 13-Week Assignment | 35-Hour Weekly Guarantee
Key Responsibilities

Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward established therapy goals
Collaborate with interdisciplinary care team to ensure comprehensive patient care
Educate patients and families on therapeutic exercises and home programs

Required Qualifications

Active Physical Therapist (PT) license in the state of Kentucky or eligibility for licensure
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Experience working in a SNF or similar clinical setting

Preferred Experience / Skills

Prior travel or contract therapy experience
Familiarity with skilled nursing documentation and Medicare guidelines
