Job Title: Registered Nurse Profession: Nursing Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 2 years in an acute hospital setting required. License: Active nursing license required. Certifications: BLS certification required. Must-Have: - Experience with acute care, ICU, MS, PICU, SDU, PCU, IMC, and Obs specialties. - Previous charge experience required. - Knowledge of case management and utilization review. - Experience in discharge planning, needs assessment, ordering DME, and developing a plan of care is required. - Familiarity with regulatory requirements such as CMS is preferred. - EPIC experience required. Description: This position is for a Registered Nurse specializing in case management. The role involves coordinating patient care and ensuring smooth transitions of care. Candidates must have experience in various acuity levels within an acute hospital setting. The position requires handling discharge planning and conducting needs assessments. Familiarity with ordering durable medical equipment and creating care plans is essential. Candidates will also need to facilitate utilization reviews and comply with regulatory standards. There is a requirement to pass a compliance EKG exam as part of onboarding. Candidates must be flexible with scheduling and adhere to punctuality requirements. They should be prepared to manage potential floating to other units as needed. This position mandates a zero-tolerance policy for tardiness and unexcused absences. The candidate must ensure they can complete all compliance requirements in a timely manner.
