We are currently seeking a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team and help improve the quality of life for our residents through exceptional therapy services.

Provide physical therapy services to residents in a skilled nursing and long-term care setting
Complete accurate and timely documentation
Develop and implement individualized therapy treatment plans
Collaborate with interdisciplinary healthcare teams to support resident progress
Follow all facility policies and procedures
Ensure compliance with all state and federal regulations and therapy standards
Promote resident independence, mobility, and overall quality of life
Current Physical Therapist (PT) license in Colorado or a compact state
Compassionate and resident-focused professional
Strong communication and teamwork skills
Organized, dependable, and motivated
Passionate about delivering high-quality rehabilitation services
