Job Overview: We are currently seeking a compassionate and skilled Home Health Speech-Language Pathologist SLP to provide personalized, one-on-one therapy in a home setting throughout Dublin and the surrounding areas. This position is ideal for a Speech-Language Pathologist who enjoys working independently while helping patients improve communication, swallowing, and cognitive function within their daily environment.

As a Home Health Speech-Language Pathologist SLP, you will work directly with patients recovering from illness or injury while developing individualized treatment plans focused on improving patient outcomes and quality of life.

Responsibilities of Home Health Speech-Language Pathologist SLP:

• Provide home-based speech therapy to patients experiencing communication, swallowing, or cognitive difficulties
• Conduct patient assessments and develop individualized treatment plans
• Observe and evaluate a patients ability to communicate, swallow, and process information
• Monitor patient progress and modify care plans as needed
• Assist patients with mouth, breathing, swallowing, fluency, articulation, and reading exercises
• Educate patients and families on therapy goals, safety practices, and self-care techniques
• Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, PT, and OT
• Maintain accurate and timely documentation of patient progress

Qualifications of Home Health Speech-Language Pathologist SLP:

• Masters or Doctoral degree in Speech-Language Pathology
• Active SLP license in the state of Virginia
• Minimum of 2 years of speech therapy experience (home health preferred)
• CPR certification and PPD/TB skin test required
• Knowledge of state and federal home health regulations
• Strong clinical judgment, critical thinking, and communication skills
• Compassionate, dependable, and patient-focused
• Ability to work independently in a home setting
