• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Shift Duration: 8 hours

• Weekly Estimated Pay: $2,120 - $2,230
• On Call / Weekend Requirement: None
• Mileage Reimbursement: Yes, between visits and from first patient to last patient

Job Requirements

• Active Registered Nurse (RN) license required
• Minimum of 1 year of nursing experience preferred, with hospice nursing experience required
• Experience using HCHB required
• Associate degree required
• Strong communication skills and a calm, flexible, and pleasant approach to patient care
• Ability to manage a full caseload while maintaining quality, compassion, and clinical accuracy

Responsibilities

• Provide compassionate hospice and home health nursing support to patients in the community
• Complete case management activities to support safe, coordinated care
• Assist with admissions, assessments, and start-of-care nursing documentation
• Prepare for interdisciplinary team meetings and coordinate care planning
• Work closely with patients, families, and care team members to support comfort, continuity of care, and quality outcomes
• Document visits and patient progress accurately in the electronic health record
• Support an average productivity expectation of 6 visits per day / 30 points per week
