Social Worker/Care Companion Location: Edwardsville, IL Company: Carease Health - IL Organization: Carease Health - IL Job Summary: At Carease Health - IL, we prioritize proactive measures for enhanced physical health and well-being by incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM) and Collaborative Care Management (CoCM) into our comprehensive preventative care program. As a Social Worker/Care Companion, you will play a crucial role in providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Key Responsibilities: Visit designated facilities as scheduled by your supervisor to conduct assessments of residents' emotional, cognitive, and psychosocial needs. Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns. Document visit notes and maintain detailed records of interactions and interventions after each visit. Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP. Understand geriatric mental health, crisis management, and psychosocial support in long-term care settings. Qualifications: Associates, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field. Strong communication, documentation, and relationship-building skills. Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
