Admissions Coordinator

Momentum at South Bay for Rehabilitation and Nursing is looking for a full-time Admissions Coordinator.

The ideal candidate will have:

• Good oral and written communication skills.
• Interview and assessment skills.
• Knowledge of entitlements and community resources.
• Knowledge of health care and gerontology issues.
• Maintain functions of the Admissions Department.

Duties and responsibilities:

• Conduct tours and provide information about Momentum at South Bay to applicants, hospital discharge planners and community agencies.
• Admit residents in accordance with established policies and procedures.
• Upon admission, responsible to complete, review and explain admission documents to residents and/or sponsor obtaining signatures as necessary.
• Complete pre-admissions in Sigma EMR.
• Other related duties as assigned by Admissions Director

Job type: Full-time

Pay: $48,000 annually

Shift: M-F
