Key Responsibilities\n- \n
- Conduct comprehensive psychosocial assessments to identify clients' mental health needs, strengths, and available resources. \n
- Provide evidence-based individual, family, and group therapy to hospice patients and their families, utilizing modalities such as cognitive-behavioral therapy and trauma-informed care. \n
- Develop, implement, and adjust individualized treatment plans that include tailored therapeutic goals and interventions designed to improve the quality of life. \n
- Collaborate with a diverse treatment team, including nurses, physicians, and other therapists, to provide comprehensive and coordinated care for clients. \n
- Offer crisis intervention, risk assessments, and safety planning when necessary to ensure clients receive timely support during critical moments. \n
- Facilitate support groups, psychoeducation classes, and workshops aimed at equipping clients and families with tools for coping with grief, loss, and related transitions. \n
- Maintain organized, accurate, and timely documentation of client interactions and treatment progress in accordance with professional and legal standards. \n
- Develop and implement community outreach programs to enhance awareness of mental health services within the hospice context. \n
